Electronic Information Enhances Patient Care for Health Service

Overview

Delivery of care by multi-disciplinary teams within the National Health Service (NHS) suffers from fragmented information and workflows. This can lead to duplication of patient records, unnecessary hospital stays, and a large volume of unplanned admissions. Working with partner D2i Solutions, Eastern and Coastal Kent Primary Care Trust (PCT) is running a pilot project to establish how technology can improve the planning and provision of care. It uses Microsoft Office Groove 2007 and interRAI Minimum Data Set (MDS) for Community Care electronic forms to supply electronic patient assessment forms, electronic care plans, and patient workspaces. Using this solution, the trust can increase the speed and accuracy of assessments and reduce fragmentation of information.
